Someone who has had a long run-in with the MAs might know for certain, but my impression is that results would normally go to the same MA. (Nobody has direct contact - all communication goes via the CO.)
This isn't regulated by how many times. It goes till the MA is satisfied they have the information needed.
Same Ma but it can go to second on appeal if you have medical opinion that disagrees with the decision of the first.

Pretty much to the same MA. If the results you submitted are not satisfactory too, it will likely mean a 2nd round of tests. By which means, you need to do something about the results; diet, medication, exercise, more specialised tests, a second opinion etc. The wait can be extruciating.

My wife was predicted to have tachycardia after initial MA and asked ECG and micro albumin- creatinine ratio.
She gave both the test and both came out to be very normal. So we submitted back our extra tests and it has been referred again to a MA today.

You don't have to be disheartened, that seems like usual process. The results go back to MA and they clear off the medicals.

we did our medicals online, both of our medicals are COMPLETED now.. we got e-mail from the nzimed site.. i called the customer centre guys and he said both of ours are acceptable standard as per MA's comments.
My question is, the status in the immigration website login shows as 'Received' still, so when this will change?
I think your CO might have to formally change this (there can be several reasons for no status change e.g. CO might be on leave or clearing out backlog of work post holiday season and many others which I don't know). Seems like good news is just round the corner. all the best

What does the medical status change to once it has been approved? From Received to what?
@SUN777: processing within NZ is different bro, here there is a medical team which takes care of all these. i was informed that CO will be allocated only in the month of March or April. So long way to go but still waiting for some good news
