    Default X-ray report slips

    Hello,
    Our CO request x-ray report slips (description of the x-ray results written and signed by the radiologist) which should have been included with our medicals. Does anyone knows what exactly is this? Is it something on the form that was not filled in or something different?

    Sounds like the X-ray result (part of medicals you submitted) was not completed or signed by the doctor.

    I remember during our medicals, doctor forgot to sign but the nurse was smart enough to check and we got everything in place before leaving the hospital.

    It's not part of the form - there's supposed to be a separate sheet, as you describe it (description of the x-ray results written and signed by the radiologist).

    AFAIK it is not part of the actual form - some CO ask for it anyway. Per form it is the GP who interprets the Xrays photos and so 'reporting' on them.

    I suggest you just ask 'naively' your CO on what basis this requested and where it is mentioned in the regulation.

    It is section J, K and L of the Medical and Chest X-ray form INZ 1007 see health forms

    We printed the entire form and the sections J, K and L were there. Thou I have no idea whether the GP filled them before sealing the entire form.

    It is the radiographer/radiologist that fills in this section not the GP, unless of course the GP is also the radiographer/radiologist.

    Isn't the radiologist ALSO expected to write something almost like a letter? - I'm going by vague memory of something I thought I'd read on somebody's thread, here. I think...

    I have to correct myself:
    Per form it is the radiologist who interprets the Xrays photos and so 'reporting' on them in sections J, K and L as per current version of the form; but still no separete reports are mentioned there.

    We had the same issue, we had done x-ray and totally remember sections J, K and L being worked by Radiologist.
    But Immi NZ complained that it was not sent through to them, we went back to medical centre with Immi NZ letter and explained them that these were missing, at first they were not willing to admit , later senior person from the medical centre agreed to re package and hand over the sealed one to us to re-send to Immi NZ ourselves (win-win for both of us) and problem sorted out.

    This generally happens on medical centres which process very low volume of visa medicals - Radiology dept is different and separate and they miss out the forms if the final person who seals them does not check all of the details have been collected and packaged.
