Default transferring irrv to new passport

I need to do this for my daughter.
her old passport expired and we have the new uk one.
do I use this form and if so, am I a person assisting her? she's 13.

Yes, that is the right form; I just got it done a fortnight ago.
And again yes when you are helping her you are officially assisting her.

I did it in person as it is close by. There is also the advantage that they check the application immediately.
I wouldn't be overly nervous - but that's up to you. E.g. I received my passport from our embassy by post down here in my letter box.

Did this about a year ago, had to wait for about an hour at the offices on Queen Street in Auckland.
