Do they want the actual data from the lab results or just the sheet that ticks off that I am HIV -ve, Hep B -ve etc?

Same with X-Rays.

Would appreciate speedy response as today is the day for us to submit!

They want the actual lab results and a radiologist report (on some type of office letterhead) in addition to the forms.

Man, the form isn't clear on that.

The other issue is that BMI measurements are in inches not cm.

The US doctor would just not fill out the form properly.

Yes. I had that problem. I was like a little annoying bug that looked over everyone's shoulder as they filled out the forms and made them cross it out and write it correctly if they did it wrong.

The lady who took my blood was hesitant to sign the photograph and paperwork.

I just kept reminding everyone who questioned me or was hesitant that the only purpose for any of these tests was for this application and it would be a useless waste of time if we didn't do it correctly.
