SFV Help!

Hi

I have faced few problems when filling the SFV form.

1. I have a long name. Don Sajeeva Madushan Perera. In my passport family name is "Perera" and Other names is "Don Sajeeva Madushan". In Our passports we don't have field called Given name 1, 2, 3 only one field called "Other Names". So what should i write for Given name 1, 2, 3.

2. What is recommended for "Identification Type". In case if it is National ID, our National ID is not in English. Im not sure whether we can translate it. So what can i do?

1. 'Given name' is just another way of saying 'other names', so those fields are where you will enter Don, Sajeeva, and Madushan.
