    Hi, some body can help me to share a sample of certificate to be collected from university that courses studied is only the medium of English.

    There isn't any special format or certificate. The letter merely has to state on headed paper that (your name) attended there between (date) and (date) as a student on (whatever course), for which the medium of instruction throughout was English.
