    Default Statutory Declaration of Birth for Primary Applicant

    Hi, Could any body explain me about " Statutory Declaration of Birth for Primary Applicant "

    As my CO requested for this in AIP letter...

    Can I do this declaration by my self without any witnesses or I need to do it by notary public?

    Please clarify...
