has anyone ever been rejected at second person check?

Oh, it is an anxious waiting game.
We finished interview on 6 April. No transcription was received. My agent said "the case officer approve your application subject to a second person check."
Is there anyone here or anyone you know has been rejected by second person?
If yes, what is the rejection reason?

quite often they are returned to the case officer for additional checks or information, but relatively rare for 2PC to completely overturn CO recommendation.
