How to apply for Indonesia Police Clearance if foreigner and outside Indonesia?

If used to worked in Indonesia in more than 12 months but the company cannot be contacted for a sponsor letter and if these are not available anymore:
a copy of Surat Tanda Melapor Diri (Certificate of Police Registration)
copy of Kartu Ijin Tinggal Sementara (Temporary Stay Permit Card in Indonesia)

According to immigration INZ website, here's how but please advice given the situation above. Thanks!


Foreign nationals who are no longer residing in Indonesia

May apply in writing through your embassy in Indonesia to:

Direktur Intelpam Polri
Kasubdit POA, Mabes Polri
Jl. Trunojoyo No. 3, Kebayoran Baru
Jakarta Selatan

You must include:

a sponsor letter from the company you worked for whilst you were in Indonesia
a copy of Surat Tanda Melapor Diri (Certificate of Police Registration)
copy of Kartu Ijin Tinggal Sementara (Temporary Stay Permit Card in Indonesia)
three passport-size photographs of yourself, red background
a copy of your passport
Your previous employer in Indonesia might be able to submit a copy of Surat Tanda Melapor Diri and Kartu Ijin Tinggal Sementara.
