Technical advisor mixed up our case with soneone else

We have just gone thru a medical waiver process. And the assessment letter have our case mixed up with some one else.

It appears rather than doing genuine assessment the TA had some prejudice and mixed up our case resulting in a medical waiver decline.

What could we do to get our case assessed properly?
