The details that are in the INZ1103 form are incorporated into the online EOI form. In short:

Submitting a manual (paper) EOI = you need to include the INZ1103

Submitting an online EOI = you've already answered the INZ1103 questions in your online EOI form, so no need to worry about the INZ1103 form.
